At PCMHD Home Health and Hospice, our team of health care professionals is committed to providing quality health care to patients in the comfort of their own homes. Being a part of our local hospital means that our patients receive the continuity of care that is so important to our patients' health. We work directly with all our local and area physicians to ensure our patients receive the best care possible.

Home Health services offered include:

At PCMHD Home Health, our goal is to help patients reach the highest level of independence possible while remaining at home.
PCMHD Home Health also treats a wide variety of wounds with the latest treatments available, including Wound Vac therapy.
We help our patients recover from illness or injury as quickly as possible so that they may return to their normal lifestyle. We also teach patients how to manage chronic conditions and control their symptoms to a level where they no longer need us. Wellness and independence are our goals.
Home Health serves the counties of Pecos, Terrell, Reeves and Ward.

HOSPICE SERVICES
We care for patients with terminal conditions in their homes, helping them to live life to the fullest. Hospice is a holistic approach to end of life care focusing on quality of life and offering physical, emotional, and spiritual comfort. The hospice goal is to help the patient on life's last journey and support family members and those close to the patient.
All of our team members have many different skills and work together to plan and coordinate care. We want to be there for the patient and family during their time of need.

Hospice provides:

●Chaplain for spiritual support
●Social Services
●Volunteers

Hospice not only takes care of the patient's physical needs, but also their emotional and spiritual needs. We also support the patient's family and caregivers through the grieving process. Hospice serves Fort Stockton, Balmorhea, Pecos, Barstow, Pyote, Wickett, Monahans, Grandfalls, Coyanosa, Imperial, Iraan, Sheffield and Sanderson

Goals of Hospice care:

●Provide a holistic approach to care for patients during life's last journey.
●Assist patients to live their lives to the fullest with the highest quality of life possible.
●Support the patient and family in dealing with emotional and spiritual needs.


HOSPICE VOLUNTEERS

Volunteers serve as a member of the Hospice Team by sharing their skills and interests in a manner that provides comfort and enriches the quality of life for those we serve. Volunteers give of their time on a regularly scheduled basis and may provide the following:
Support services- companionship, conversation, active listening, bedside sitting and letter writing. Sharing of Hobbies and special interests, such as reading, gardening, listening to music, sports travel, crafts etc. Assisting with errands grocery shopping, transportation for banking and other miscellaneous appointments or social outings. Homemaking tasks - light housekeeping, meal preparation or child care. Yard maintenance and minor home repairs.
Administrative support; filing, mailing reception assistance, clerical support or working with special events such as memorial services, fundraising and public relation events. Trained Volunteers may be used in community education and public speaking.

HOSPICE CARE IN THE NURSING HOME

In a nursing home setting, hospice helps patients, families, and nursing home staff by providing:

●Regular visits by a hospice Registered Nurse to the nursing home.
●Consultations by a specialized hospice physician as needed.
●Expert management of pain and other symptoms, such as problems breathing or swallowing.
●Education for nursing home staff, patients and families about patients' condition, symptoms, medications, and how to best care for patients' medical needs during this phase of their illness.
●Emotional and spiritual support for both the patient and their family during this phase of life. This includes help for the family before and after the patient dies.
●Provides medications and supplies related to the patient's terminal illness
●Coordinating the patient's care and medications across all of the patient's medical providers, including the patient's own doctors, hospice doctors, hospice nurses, hospice aides and all nursing home staff.

Hospice care while living in a nursing home for some patients with a terminal illness, their "home" is in a nursing home.

Keep in mind that Hospice benefits do not usually cover the cost of housing a patient in a nursing home or assisted living facility (also known as daily room and board costs). However, these costs may be covered by traditional Medicaid benefits.
